Our books are now open - new patients welcome
Nelson Family Medicine have opened their books again. We are now in a position to welcome new patients.
To make this process as easy as possible for you we’ve aligned with IntelliMed to provide an online enrolment form for all those wanting to become Nelson Family Medicine patients. Below that is a link to a PDF version of our Enrolment Pack. This PDF has all the forms to complete registration and helpful and relevant information regarding the process.
Consultation fees
People with a Community Services Card (CSC) and their dependants aged 14 to 17 years who are enrolled with Nelson Family Medicine will pay less than $20 for a standard visit with a nurse or doctor.
